Exciting opportunity to join our world-renowned pediatric burn hospital with Harvard affiliation and pioneering research.
We are currently seeking qualified and talented candidates for a Registered Nurse/ Care

Manager position in our Boston location. This is a 36 hours, week -day position.   


Position Summary:

The RN Care Manager position requires a thorough understanding of clinical, financial, administrative and psychosocial components of care management. Also responsible for providing assessments, planning, interventions and follow-up for patients/family members.

Key Responsibilities:

> Advocates for the patient/family while adhering to the overall treatment plan
> Provides discharge/transition assessments, identifying barriers that affect length of stay; identifying barriers to successful clinical outcomes.
> Works closely with providers, patient/family to facilitate medication management/adherence through proactive follow-up.
> Performs developmental touch point assessments for patients with children with medical complexity based on stratification.
> Provides anticipatory guidance about health care benefits to minimize over/under utilization and decrease fragmentation of care. 
> Daily contact with inpatients as well as the clinical team, ensuring timely care and services, identify needs and develop action plans.
> Reviews authorization for services, orders and documentation to support medical necessity and level of care, prior and during encounters, in conjunction with the in house team.


Job Requirements

Bachelors of Nursing with 5 years plus Pediatric Case Managment experience strongly preferred, along with the ability to coordinate interdisciplinary and interagency care. Current Case Management Certification required; if not certified in case management, certification by CCM or ACM will be required within 6 months of hire.
