RN CARE MANAGER OPPORTUNITY

 

SHRINERS HOSPITAL FOR CHILDREN – CINCINNATI*

 

*Hospital will be transitioning to Dayton Children's to be a hospital within a hospital model of care in early 2021.

Shriners Hospitals for Children- Cincinnati is seeking a part-time RN Care Manager to join our Care Management team!

 

The RN Care Manager position requires a thorough understanding of clinical, financial, administrative, and psychosocial components of the healthcare continuum The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Hospitals for Children (SHC) across the continuum of care. These services are provided face to face or telephonically to facilitate the creation of and adherence to, a treatment plan and optimize patient health. In conjunction with the interdisciplinary team, this position proactively addresses potential or actual clinical and/or psychosocial needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated.

Work in Cincinnati until our move to Dayton in early 2021

BSN required, MSN preferred

Ohio RN License required

3 years RN experience with 1+ year of pediatric case management experience preferred

Case Management Certification (CCM or ACM) is required or obtained within 6 months of hire or eligibility

Experience with Utilization Management in a hospital setting preferred

Strong communication skills-both oral and written

Excellent customer service and service recovery skills

Desire for and potential for growth and professional development. Ability to manage conflicting priorities.

Empathy, caring and compassion for children with complex medical conditions and their support systems

Strong computer skill with proficiency with Microsoft Office Suite

Willingness to support efforts to maximize the efficiency and effectiveness of the electronic medical record.

Bilingual in English and one additional language is preferred
